Certified copies for all vital statistics records (i.e. Birth, Marriage, Death) are $10 for the first copy and $5 for each additional copy ordered at the same time as original request.

A completed Request for Vital Record form and proper I.D. is required for issuance of all certified copies of vital statistics records.
